You can now complete most of the registration for our practice here online as an option
As long as you live within the practice area you are welcome to register with the Practice. Our receptionists will give you forms to complete.
A Health Visitor will contact parents of under fives. You will be registered with the practice rather than an individual GP.
You have the right to ask to see a particular GP but we cannot guarantee this.
You may print off the registration form by clicking HERE to download, fill it out and bring it in with you to the practice OR come to the surgery to collect a registration pack OR complete online registration forms via the links below OR there is a new option to register via the NHS APP online service. Please be aware that if you register via the app or NHS online - registration will be subject to a manual address check here at the surgery where we will check in detail that your address fits into our Practice boundar. South of Ham Drive & Weston Mill Cemetery, east of Outland Road / Alma Road, taking the boundary directly south from Pennycomequick Roundabout to the Octagon and Millbay.

At St Levan Surgery, we DO NOT offer the DES (Direct Enhanced Service) that includes registering out of area patients without home visiting duties.
We wish to make clear that we will only register patients living within our Practice area. Thank you for your co-operation.
Temporary Patient Registrations
If you are ill while away from home you may be able to register as a temporary patient to receive care from our practice.
This will allow you to be on our practice list and still remain a patient of your permanent GP.
