If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness to Work’ Certificates.
Please contact the surgery to arrange this.
PLEASE NOTE SICK/ FIT NOTE CERTIFICATES CAN TAKE UP TO 72 WORKING HOURS/ 3 WORKING DAYS TO PROVIDE
If you have already had a Sick Note (Fit Note) for this illness your Doctor may not need to see you to issue an additional Sick Note. Please contact the surgery.
Use form SC2 if you are an employee, you’ve been sick for 4 or more days in a row and need to claim SSP from your employer.
When you have filled in the form give it to your employer, it will help them decide if you can get SSP. Keep a copy for your own information.
